When you’re prescribed or supplied critical electrical medical equipment, you should be provided with a Notice of Potential Medically Dependent Consumer Status by your District Health Board, private hospital or General Practitioner.
If you or someone at your premises is dependent on mains electricity for critical medical support such that the loss of electricity may result in loss of life or serious harm you should notify us that you or such other person is a medically dependent consumer in the following manner: You must provide us with a Notice of Potential Medically Dependent Consumer Status Form which is available from your primary healthcare provider and which has been signed by a medical professional.
