Change Orders for WPCLF projects should be submitted to the Division of Environmental and Financial Assistance (DEFA).
Public Schools shall maintain a sick leave bank for the benefit of all employees as outlined in Stillwater Public Schools Board Policy DEFA.
One copy of the Change Order is to be submitted to Ohio EPA – DEFA for review and confirmation of the acceptability of the change.
After the change order is executed, a minimum of three copies are to be sent to Ohio EPA ‐ DEFA for final review.
Health Departments should submit change orders electronically to the DEFA Engineer who reviewed and approved their project.
