Coordinated Home Care Program definition
Coordinated Home Care Program means an organized skilled patient care program in which care is provided in the home. Care may be provided by a Hospital's licensed home health department or by other licensed home health agencies. You must be homebound (that is, unable to leave home without assis tance and requiring supportive devices or special transportation) and you must require Skilled Nursing Service on an intermittent basis under the direction of your Physician. This program includes Skilled Nursing Service by a registered professional nurse, the services of physical, occupational and speech therapists, Hospital laboratories and necessary medical supplies. The program does not in clude and is not intended to provide benefits for Private Duty Nursing Service. It also does not cover services for activities of daily living (personal hygiene, clean ing, cooking, etc.) Congenital or Genetic Disorder ……means a disorder that includes, but is not limited to, hereditary disorders. Congenital or Genetic Disorders may also in clude, but are not limited to, Autism or an Autism Spectrum Disorder, cerebral palsy, and other disorders resulting from early childhood illness, trauma or injury. Continuous Ambulatory Peritoneal Dialysis Treatment ……means a continuous dialysis process using a patient's peritoneal membrane as a dialyzer. Copayment ……means a specified dollar amount that you are required to pay towards a Covered Service. Coverage Date ……means the date on which your coverage under this Certifi cate begins.

The program does not include and is not intended to provide benefits for Private Duty Nursing Service or Custodial Care Service. It also does not cover services for activities of daily living (personal hygiene, cleaning, cooking, etc.).

If you are an Inpatient in a Hospital, Skilled Nursing Facility or other eligible facility or a patient in a Partial Hospitalization Treatment Program or Coordinated Home Care Program at the time that your coverage under this Certificate is terminated, the benefits of this benefit section will be provided until you are discharged from the Hospital or other Provider or until your Hospital days are exhausted, whichever occurs first.
