Contract Summary Sheet definition

Contract Summary Sheet means the summary of terms of a Customer Agreement and the Health Benefits Plan along with any pertinent attachments and an acceptance/rejection form prepared and compiled by MCHP and transmitted to a Group pursuant to which a Group shall accept or reject the applicable Customer Agreement. A Contract Summary Sheet may also include updates to Customer Agreements from time to time.
