Continuing Health Care definition

Continuing Health Care. (CHC) shall mean the name given to a package of care that is arranged and funded solely by the National Health Service (NHS) for individuals who are not in hospital and have been assessed as having a primary health need.
Continuing Health Care is the name given to a package of services which is arranged and funded by the NHS for those people who have been assessed as having a primary health need (assessed by its nature; intensity; complexity; and unpredictability). Continuing Health Care can be received in any setting including a person’s own home or in a care home;

Examples of Continuing Health Care in a sentence

  • There is no charge for Intermediate Care or Continuing Health Care services.

  • In certain circumstances, where the NHS is providing Continuing Health Care to place you within the Care Home, there may be a requirement for an additional contribution from either you or a Third Party to contribute to the Fees for enhanced levels of accommodation.

  • The Chief Nurse leads the Continuing Health Care Team and assumes a consultative and advisory role in the clinical and operational aspects of the CHC team.

  • Under this section 75 agreement, NHS Oldham CCG and Oldham Council will jointly commission, contract and monitor services for adults, currently funded separately through the CCG’s Continuing Health Care budget and the Council’s community care budget.

  • Current arrangements will continue in the interim period, whereby the Council records transactions for residential care within the Council system and the CCG records the transactions for Funded Nursing Care and Continuing Health Care with the CCG financial system.

  • With regard to Continuing Health Care and Funded Nursing Care the following Scheme of Delegation will apply.

  • Personal health budgets will be available by right for people with NHS Continuing Health Care, with anyone that has a long term condition, disability or mental health need also able to benefit.

  • The Continuing Health Care Benefits are not subject to liquidation or exchange for any other benefit.

  • The pathway complies with and promotes the use of the national and local guidance, policies and procedures in relation to Fair Access to Care Services (FACS), Continuing Health Care (CHC),Funded Nursing Care (FNC) and Jointly Commissioned Care.
