Continuing Care Services definition

Continuing Care Services means the continuum of care, ranging from independent living to assisted living to nursing home care, provided pursuant to a contract for the life of the individual purchasing the services or for a period of not less than one year.
Continuing Care Services has the meaning set out in Section 3.16.6.
Continuing Care Services means skilled nursing and assisted living services provided at the Health Care Facility as more particularly described in Paragraph 7(i) of this Agreement.

Examples of Continuing Care Services in a sentence

  • For the purpose of providing adequate PHN staffing of weekends/holidays and promoting flexibility and job satisfaction for the PHNs, trading weekends/holidays is an allowed part of the staffing system in Continuing Care Services.

  • In 2000, this Agreement was amended to include all Registered Nurses employed in Continuing Care Services.

  • Continuing Care Services will follow outpatient on-call language.

  • As part of the treatment in IOP, inmates will be introduced and encouraged to develop a long-term plan that will include IOP, Continuing Care Services while incarcerated and post-release reentry services upon return to the free-world community.

  • Immediately upon successful completion of the IOP, inmates will be given the opportunity to enter Continuing Care Services.

  • For the purposes of addressing the mutual interest of appropriate pay for after-hours call (standby) and callback within the Continuing Care Services Department and for the Nurse Practitioners who work with the Long Term Care Program, there is an additional stipend of $700 per quarter year for each nurse practitioner who participates in CCS after-hours call (standby) to compensate for the additional work expectations.

  • It is understood that some departments (for example, Continuing Care Services) have unique health and safety issues.

  • Continuing Care Services - For inmates who have successfully completed the 208-hour IOP, the treatment gains they will have made will be supported and reinforced through Continuing Care Services.

  • The Continuing Care Services will follow the parameters outlined in ODRC and OhioMHAS policies.

  • Notwithstanding any other provision of this Agreement and subject to the terms of the Continuing Care Services set forth in Schedule A, Hythiam in no event or circumstance is or shall be responsible for any costs of, or related to, patient care provided by Group, or extended or unanticipated care required for patients treated using the Licensed Technology.
