The following information is to be included in the notification: Name of beneficiary; Beneficiary ID number (Medicaid/MI Child); Consumer ID if no beneficiary ID; Date, time, and place of death; Preliminary cause of death; Contact persons’ name and Email address.

You agree to provide CR with basic information about your Chex Systems report at signup including, but not limited to, your full legal name, date of birth, address, telephone number where you can be reached, Chex Systems Consumer ID, the method in which you opened the account in question and email address.
