Efforts will be made to provide Community Integration and Employment Specialists with a substitute when in-service training occurs during the work day.
No individual shall be transferred from a Training Center to a nursing home or congregate setting with five or more individuals unless placement in such a facility is in accordance with the individual’s informed choice after receiving options for community placements, services, and supports and is reviewed by the Community Integration Manager to ensure such placement is consistent with the individual’s informed choice.
The case shall be referred to the Community Integration Manager and Regional Support Team in accordance with Sections IV.D.2.a and f and IV.D.3 below, and such placements shall only occur as permitted by Section IV.C.6.
Where discharge does not occur within 3 months of selecting a provider, the PST shall identify the barriers to discharge and notify the Facility Director and Community Integration Manager in accordance with Section IV.D.2 below, and the case shall be referred to the Regional Support Teams in accordance with Section IV.D.3 below.
The State shall ensure that information about barriers to discharge from involved providers, CSB case managers, Regional Support Teams, Community Integration Managers, and individuals’ ISPs is collected from the Training Centers and is aggregated and analyzed for ongoing quality improvement, discharge planning, and development of community-based services.
Adult family home providers who offer and provide support to clients with an assessed need for support to access and participate in the community through a contract with a PACE organization will receive a rate inclusive of the Community Integration rates described above.
Each Community Integration and Employment Specialist will be provided two in-service days per year for which he/she will be released from his/her regular duties in order to attend.
After ten (10) work days, if a substitute has not been found, all Habilitation Workshop Specialist II’s (Community Integration Specialists) and Registered Adult Service Aides affected by the absence will receive an additional stipend of thirty dollars ($30.00) per day, effective on the eleventh workday of the absence.
