Collaborative Network definition

Collaborative Network means two or more healthcare organizations that agree to make their data available for querying as a single network and accessible only to the healthcare organizations participating in the collaboration, through the use of the TriNetX Live Platform.

Collaborative Network is used in this document to define a group of agencies or organizations committed to the operation of a CRC for providing transitional support and for delivering public awareness, information, referral, assistance, and access to consumers about the array of long-term services and supports. In Wake County, the “Collaborative Network” for the CRC is referred to as the Wake CRC Advisory Committee. The Advisory Committee is appointed by the GOLD (Growing Older, Living with Dignity) Coalition, an ongoing public-private working group responsible for the monitoring and implementation of Wake County’s Aging Plan. The Wake CRC Advisory Committee functions in an advisory capacity to the GOLD Coalition.
