Collaborating prescriber definition
Collaborating prescriber means the physician, advanced practice nurse or physician assistant who is a party to a collaborative pharmacy practice agreement, who has a direct provider/patient relationship with the patient served by the agreement and who has prepared the patient specific, drug specific, disease or condition specific plan of care based on a physical examination of the patient where required under these rules.
