Coded Service Identifier(s) definition

Coded Service Identifier(s) means a listing of descriptive terms and identifying codes, updated from time to time by CMS or other industry source, for reporting Health Services on the CMS 1500 or CMS 1450/UB-04 claim form or its successor as applicable based on the services provided. The codes include but are not limited to, American Medical Association Current Procedural Terminology ("CPT®-4"), CMS Healthcare Common Procedure Coding System ("HCPCS"), International Classification of Diseases, 10th Revision ("ICD-10"), National Uniform Billing Committee ("Revenue Code") and National Drug Code ("NDC") or their successors.

Examples of Coded Service Identifier(s) in a sentence

  • Anthem shall use best efforts to update all applicable Coded Service Identifiers when they are released by the applicable authority.

  • Coded Service Identifier(s) used to define specific rates are updated from time to time to reflect new, deleted or replacement codes.

  • Unless otherwise instructed, or required by state or federal law, Provider shall submit Claims to Plan, subject to any applicable HIPAA requirements, using appropriate and current Coded Service Identifier(s), within one hundred eighty (180) days for HMO Colorado and three hundred sixty five (365) days for all other products from the date the Health Services are rendered or Plan will refuse payment.

  • Anthem shall use commercially reasonable efforts to update all applicable Coded Service Identifiers within sixty (60) days of release by CMS or other applicable authority.
