Clean File definition

Clean File for a physician Provider means the credentialing file for Provider that meets the criteria set forth for employment under Tenet’s Physician Employment Arrangements policy (L-10), the criteria for which is set forth as Exhibit A. “Clean File” for a non-physician Provider means the credentialing file for Provider meeting the same criteria for a physician Provider, as applicable for a non-physician Provider.
Clean File means a credentialing or re-credentialing file that meets the minimum requirements set forth in this Plan. A Clean File must also have had a Clean File review during initial credentialing or the prior re-credentialing cycle.
Clean File means a practitioner file that meets the established criteria and does not contain any adverse findings.
