Certification Regarding Debarment and Suspension definition

Certification Regarding Debarment and Suspension means the Department is prohibited from contracting with a provider for $100,000 or more in federal monies if the provider has been debarred, suspended, declared ineligible or voluntarily excluded from receiving federal funds. This portion is also true of providers who audit federal programs, regardless of the amount of federal moneys involved in their contracts. The Certification Regarding Debarment and Suspension form must accompany Requests for Proposal, Invitations to Bid, Invitations to Negotiate, and contracts and states that the provider is not ineligible to receive federal funds.
