CDB Enrollment Form definition
CDB Enrollment Form means that certain Commercial Digital (online) Banking Enrollment Form pursuant to which the Customer requests, modifies or cancels CDB offered in accordance with the Master Agreement and this Addendum.

In order for Customer to use CDB, Customer must have at least one open Account and an Authorized Representative must execute this Addendum and the CDB Enrollment Form.
Upon acceptance of this Addendum and the CDB Enrollment Form by Bank, the Master User designated by the Authorized Representative in the CDB Enrollment Form will receive a security code and a secure link to establish the Master User's Access ID.
