Case Notes definition

Case Notes. Logs and/or sign-in sheets, progress notes, monthly reports, summary notes and/or any other written or electronic documentation completed by the Direct Service Provider to support that the covered service was provided to the Service Recipient. Case Notes must include the following minimum elements: service code or name; name(s) of the direct service provider(s); client and service recipient name; the date, actual start time, actual end time, duration, location of the service; intervention; summary of the activity engaged in; Service Recipient’s response to the Covered Service; Direct Service Providers signature and signature date and any other elements as required by Purchaser Policy or Procedure. System and other requirements for electronic Case Notes and other electronic service documentation are listed elsewhere in this Agreement.

Case Notes means the narrative section of the Participant Case Record that document service details.

Examples of Case Notes in a sentence

  • Agency must update Demographics, Annual Review, Services, Case Notes, Encounter Tab and all requisite Custom Tabs in the DOH approved data system, as appropriate, within five (5) business days from Client Interaction or on behalf of Client Agency must Track and report within the DOH-approved data system any and all Performance Measures related to this Service Category as directed by DOH Quality Team.

  • Documents include but are not limited to: Interview Reports, Arrest Documents, Evidentiary Documentation, Case Notes, Investigation Reports, photographs and miscellaneous correspondence.

  • Targeted population: Persons living with HIV Deliverables/Measures: Number of PLWH to be served: 200 Reporting: • Agency must update Demographics, Annual Review, Services, Case Notes, Encounter Tab and all requisite Custom Tabs in CAREWare, as appropriate, within five (5) business days from Client Intake, identified change in Client Status, delivery of a support service or benefit, or Interaction with or on behalf of Client.

  • If a customer declines follow up services, this must be recorded in the customer’s case file under Case Notes and in CalJOBS.

  • Resources provided must be clearly documented on Case Notes and TR.

  • Targeted population: Persons living with HIV Deliverables/Measures: Number of PLWH to be served: 12 Reporting: • Agency must update Demographics, Annual Review, Services, Case Notes, Encounter Tab and all requisite Custom Tabs in CAREWare, as appropriate, within five (5) business days from Client Intake, identified change in Client Status, delivery of a support service or benefit, or Interaction with or on behalf of Client.

  • Proof of this work will be shown with Case Notes and is subject to management approval.

  • V-Track has additional features, such as Security Threat Group classification, Case Notes capability, and an extensive list of reporting capabilities.

  • Class E Notes has the meaning assigned to such term in the Indenture.
