Care Team definition

Care Team means a Participating Practice-based team consisting of an eligible provider, one or more Care Managers or Care Coordinators, and/or other clinical support or allied professional staff within the Participating Practice who jointly manage care for the entire patient panel as well as distinct subsets of patients.
Care Team. ((includes)) means the client and everyone in- volved in his or her care. The care team ((can)) may include family, friends, doctors, nurses, long-term care workers, social workers, and case managers. The role of the care team is to support the client's well-being ((of the client,)). However, the client directs the care plan.
Care Team means a group of individuals responsible for the development of each patient's care plan. The care team shall consist of, but not be limited to, the physician or podiatrist, pertinent members of the agency staff, the patient and member of his/her family.

Care Team means the coordinated team of staff and healthcare professionals organized around a particular patient and family, whose common goal is the delivery of safe and effective care through patient and family engagement;
Care Team. To ensure that no student “falls through the cracks” a standing committee known as the Care Team—comprised of staff members with particular experience and passion for student support—meets regularly to monitor progress and to advocate for students.
