Care Team definition

Care Team means the client and everyone involved in ((his or her)) the individual's care. The care team may include family, friends, doctors, nurses, long-term care workers, social workers, and case managers. The role of the care team is to support the client's well-being. However, the client directs the care plan.
Care Team means an individual or an organization used or assembled by or through Customer or on behalf of Customer or in conjunction with Customer to assist Customer, directly or indirectly, in providing to a Health Seeker (defined below) the assistance he or she seeks or requires.

Care Team means the coordinated team of staff and healthcare professionals organized around a particular patient and family, whose common goal is the delivery of safe and effective care through patient and family engagement;
