Care conference definition
Care conference means a meeting of the service provider, participant, the family or caregiver of the participant, and the interdisciplinary team for the purpose of discussing, identifying, addressing, implementing, and reviewing plans to meet needs regarding the participant’s care.
Care conference is a meeting of health care professionals who are members of the Care Team of an Attributed Life participating in the OneCare Care Coordination Program where care planning is actively evaluated and conducted and documented in Care Navigator or other mutually agreed to care coordination software. Participant and Preferred Provider eligibility for the OneCare Supplemental Care Coordination Payments described in Exhibit A2 requires the documentation of care coordination activities in OneCare’s Care Navigator software system, or other mutually agreed-upon method of documentation in compliance with the OneCare Clinical Model.
