Breast Surgery definition

Breast Surgery. The Member must provide documentation in the form of a written psychological assessment from at least one qualified behavioral health provider experienced in treating Gender Dysphoria. The assessment must document that the Member meets all of the following criteria:  Has persistent, well-documented Gender Dysphoria;  Has the capacity to make a fully informed decision and to consent for treatment;  Must be 18 years or older; and  If significant medical or mental health concerns are present, they must be reasonably well controlled.

Examples of Breast Surgery in a sentence

  • Reconstructive Breast Surgery Covered in accordance with TCA 56-7-2507, which requires coverage of all stages of reconstructive breast surgery on a diseased breast as a result of a mastectomy, as well as surgical procedures on the non-diseased breast to establish symmetry between the two breasts in the manner chosen by the physician.

  • Postmastectomy Care and Reconstructive Breast Surgery Postmastectomy Care‌ Medically Necessary Inpatient Care for the period of time determined by the attending Physician and the Member, following a mastectomy, a lumpectomy, or a lymph node dissection for the treatment of breast cancer.

  • Reconstructive Breast Surgery resulting from a mastectomy is covered.

  • All other 18 surgery performed for cosmetic purposes is excluded, except as specifically provided for treatment after 19 a mastectomy (see Reconstructive Breast Surgery).

  • Under this Lot the supplier will provide the below packs (this list is not exhaustive): o Minor Ear o Major Ear o Nasal o Adenotonsillectomy o Head and Neck o Minor Plastic Pack o Major Plastic Pack o Cleft o Plastic Hand Surgery o Burns Pack o Breast Surgery Pack o Dental o Maxillofacial surgery TERMS AND CONDITIONS / ACTIVITY BASED INCOME (ABI) The terms and conditions of this Agreement and any resulting call-off contract is appended to the ITT.

  • Benefits will not be provided for cosmetic surgery (except as specifically provided for Reconstructive Breast Surgery, Reconstructive Surgery and services for cleft lip or cleft palate or both, as listed above) or other services primarily intended to correct, change or improve appearances.
