Benefit Booklets definition

Benefit Booklets means the booklets, policies, contracts (and attachments thereto), Evidence of Coverage certificates and other benefit information that are incorporated into this document by reference and listed in Appendix B.

Benefit Booklets means the booklet(s) provided by BCBSNC to Plan Sponsor and/or Plan Administrator that is finalized pursuant to Section 6.1 of this Agreement containing a description of the benefits to be administered by BCBSNC under this Agreement. Plan Sponsor and/or Plan Administrator will receive a new Benefit Booklet each Renewal Period. Before the Benefit Booklet for the initial Agreement Period or any Renewal Period is finalized pursuant to Section 6.1 of this Agreement, the term “Benefit Booklet” for such period shall refer to the Sample Booklet, as modified by the Group Application and the Benefit Highlight, as further described in Section 6.1.

  • Blue Cross NC will deliver a paper copy of the Benefit Booklets to Subscribers and custodial parents or legal guardians under a court or administrative order upon their request.
