basic monthly fee means the total amount that a licensee is authorized to charge to a subscriber on a monthly basis for provision of the basic service to an outlet to which a television receiver, FM receiver, channel converter or other terminal device may be connected in the subscriber’s residence or premises. The monthly fee shall be automatically charge on the 1st, 15th, or 30th of each month (date to be selected by the patient), beginning with the month following enrollment. The basic monthly fee will not be prorated. • Immunization Costs: Patient shall pay the cost of any vaccine or immunization (except influenza) at the time of service. If the Principal Patient has not provided advance authorization to Peaceful Balance Health & Wellness, L.L.C to withdraw the vaccine or immunization from the credit card or bank account on file, each patient shall be responsible for these costs. • Cancelled/No-Show Appointments: Principal Patient authorizes Peaceful Balance Health & Wellness, L.L.C to charge their credit card or bank account on file, $25 for each appointment cancelled with less than 24-hour notice, by any patient covered under this plan. In addition, Principal Patient authorizes Health You to charge $50 for each no-show appointment.
