Attributed Population definition

Attributed Population means the attributed population of the ▇▇▇ ▇▇▇ at maturity, as identified by the Ministry;
Attributed Population means the Patients included in the Total Cost of Care calculations for which the IHP is accountable.‌ Attribution means the process, described in section 12.3, of determining which Patients are assigned to a particular IHP.‌ Claims Run-out means the period of time between the date a service is rendered and the date the claims or encounter data record is considered complete.‌ Contract means this Contract, its terms and conditions, attachments, documents incorporated by reference under the terms of this Contract, and any future modifying agreements made pursuant to section 11.5 of this Contract.‌ Day means calendar day unless otherwise specified (for example, business day).‌ Fee For Service (FFS) means the Minnesota Health Care Programs payment method whereby a health care provider is paid directly by DHS for each service rendered.‌
Attributed Population means the Patients included in the Total Cost of Care calculations for which the IHP is accountable.

Examples of Attributed Population in a sentence

  • IHP may reconcile its patients to its Attributed Population list.‌ In the event that IHP believes an Attributed Population list contains errors, IHP must provide notice and supporting data to the STATE, according to error report specifications provided by the STATE, no later than sixty (60) days after the receiving the Attributed Population list associated with the settlement calculation.

  • The Interim Payment will be calculated no later than five (5) months following the end of the Performance Period based on:‌ The final Base Period TCOC based on the claims incurred during the Base Period by the Attributed Population in the final Base Period Attributed Population.

  • Clinical Risk‌ The PBP Eligible Attributed Population is defined in section 12.3, Patient Attribution Method.

  • The STATE will provide clinical data, enrollment data, ACG risk adjustment output and claims-level data outlined in (1) or (2) below for the IHP’s Attributed Population monthly throughout the term of this Contract, unless otherwise mutually agreed by the Parties in writing.

  • The interim Performance Period TCOC based on the claims incurred during the Performance Period by the Attributed Population in the interim Performance Period Attributed Population.

  • COVID-19 symptoms means fever of 100.4 degrees Fahrenheit or higher, chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea, unless a licensed health care professional determines the person’s symptoms were caused by a known condition other than COVID-19.
