Attestations definition
Attestations means collectively a Seller Attestation and an Engineer Attestation.
Attestations means documents that EH representatives and EPs must sign in order to receive incentive payments. The attestation includes language that the signee agrees to the terms of the program and verifies its accuracy. For the New Mexico Medicaid EHR Incentive Program, the attestation is generated by the New Mexico State Level Registry (SLR), a web-based portal where EPs and EHs enter required data for the New Mexico Medicaid EHR Incentive Program. The signee must upload the signed and dated attestation into the SLR and then mail in the original signed and dated attestation.
