Examples of Approved Site(s) in a sentence
For all Approved Sites, notify BCBSM in writing within thirty (30) days of changes in Provider’s business including changes in business name, tax name, primary or branch locations, phone number, business structure, range of services offered, or National Provider Identifier.
Provider must have a governing board that is legally responsible for the total operation of all Approved Sites, and for ensuring that quality medical care is provided in a safe environment.
Facility will only bill BCBSM for services provided by Approved Sites.
BCBSM must give prior written approval to the establishment of new facilities before such facilities can become Approved Sites.
Developer may request to substitute an alternate Authority Housing Lot for any of the Approved Sites or to make material changes to the size or boundaries of an Approved Site, with a brief explanation as to why Developer is requesting the substitution or change.
