Application records definition
Application records means, for the purposes of this regulation, any written or electronic application form; any enrollment form; any order or document used when services are contracted; any document used to add coverage under an existing policy; any questionnaire, telephone interview form, paramedical interview form; or any other document used to question or underwrite an applicant for a policy issued by an entity or person or for any declination of coverage by an entity or person.
