The invoice forms prepared by CONTRACTOR, and approved as to form by COMMISSION, shall include, at a minimum, the following information: CONTRACTOR name, Program Name, Report Period, Invoice Amount, Approved Budget by Category, Year-to-Date (“YTD”) Actual Total, Approved Budget Total and Budget Balance.
