Alpha Report definition

Alpha Report. On a monthly basis, the Employer shall furnish electronically to the Union and sub-local officers an alphabetical listing, by department of all employees represented by the Union. The report shall notate changes in employment status. The list shall be provided by the close of business by the last working day of the month. The list shall contain: • Employee’s name • Home addressHome phone number • Work email addressWork phone numberWork location • Department • FTE status • Employee ID number • Hire dateAnniversary date • Classification

Alpha Report. On a monthly basis (no later than the tenth [10th] of the month), the Employer shall furnish electronically to the Union and sub-local officers an alphabetical listing, by department of all employees represented by the Union. The report shall notate changes in employment status (e.g. new hires, separations, terminations, transfers, promotions). The list shall be provided by the close of business by the last working day of the month. The list shall contain the following information for each employee:  Name  Date of BirthDate of HireHome address  Home email address when provided  Home phone number  Work email address  Work phone numberWork location  Department  Gender as reported by the employee  Race as reported by the employee  Ethnicity as reported by the employee  Job TitleFTE status  Employee ID number  Hire dateAnniversary date  Classification If the Employer has such information in its records, the list will also include:  Cellular phone number  Home email address
