Alert Response means a set of predefined actions performed by the NOC in response to an Alert;

  • Typical services to be provided at the unit rate identified in the Contractor’s Area Plan, as updated, include the following: Adult Day Health Care; Emergency Alert Response; Gerontological Counseling; Health Support; Home Health Aide; Medication Management; Mental Health Counseling/Screening; Nutrition Counseling; Occupational Therapy; Personal Care; Physical Therapy; Skilled Nursing Services; Specialized Medical Equipment, Services, and Supplies; and Speech Therapy.
