  • Upon approval by HPG, which shall not be unreasonably withheld, HPG will permit such additional Facilities to participate under the Program, after Exhibit B has been amended to reflect any Facilities so approved by HPG, and a signed GPO Affiliation Certificate for each additional Facility has been received by HPG.

  • Participant and its Locations shall also have the right to purchase products and services under any other Vendor Contracts CPG may have for CPG Categories that are not listed in Exhibit D and which are not Non-committed Categories, provided the Participant signs an amended or new GPO Affiliation Certificate for its Locations (if so requested by CPG), as well as an amendment to this Agreement to update the list of Committed Categories for Exhibit D.

  • Participant and CPG hereby agree that upon Participant’s provision of each updated electronic file or acceptable alternative listing of Locations, such will be deemed to be an amendment to Exhibit B to this Agreement as well as to the GPO Affiliation Certificate signed by Participant.

  • GPO Affiliation Certificate for Facilities (a copy of which is attached as Exhibit C) shall be executed for access to the Program.
