Reference is made to that certain Warrant Agreement between LabStyle Innovations Corp., a Delaware corporation (the “Company”), and __________ (the “Purchaser”), dated as of __________ (the “Warrant Agreement”), attached hereto as Exhibit A, pursuant to which the Company granted the Purchaser a warrant to acquire up to an aggregate of __________ shares of Common Stock, par value $0.0001 per share, of the Company at a per share exercise price of $0.24 (subject to certain adjustments as provided in the Warrant Agreement) (the “Warrant”).
