THIS FOURTH SUPPLEMENTAL INDENTURE (the “Fourth Supplemental Indenture”), dated as of August 23, 2013, is by and among LifePoint Hospitals, Inc. a Delaware corporation (the “Company”), the guarantors named herein (the “Guarantors”) and The Bank of New York Mellon Trust Company, N.A., as trustee (the “Trustee”) under the Indenture referred to below, as defined below.

Amendment No. 2, dated as of August 23, 2013 (“Amendment No. 2”), by and among LIFEPOINT HOSPITALS, INC., a Delaware corporation (the “Borrower”), the Lenders party hereto, CITIGROUP GLOBAL MARKETS INC., as administrative agent for the Lenders (the “Administrative Agent”), and CITIGROUP GLOBAL MARKETS INC., as lead arranger, to the Credit Agreement dated as of July 24, 2012 (as amended by Amendment No. 1 dated as of February 6, 2013 and as amended, extended, amended and restated, supplemented or otherwise modified from time to time, the “Credit Agreement”), among Borrower, the financial institutions listed on Schedule 2.01 thereto, as such Schedule may from time to time be supplemented and amended (the “Lenders”) and the Administrative Agent, BANK OF AMERICA, N.A. and BARCLAYS BANK PLC, as co-syndication agents, and CITIGROUP GLOBAL MARKETS INC., MERRILL LYNCH, PIERCE, FENNER & SMITH INCORPORATED and BARCLAYS BANK PLC, as joint lead arrangers and joint bookrunners.
