THIS AMENDED AND RESTATED INDEMNIFICATION AGREEMENT (this “Agreement”) dated as of August 11, 2011, is made by and between COMBIMATRIX CORPORATION, a Delaware corporation (the “Company”), and (“Indemnitee”).
