The following enrollment & release agreement (“Agreement”) is effective as of the date written on this form, and shall remain effective for as long as the Swimmer (“Swimmer”) is enrolled in a Calphin Aquatic Club (“CALPHIN”) (formally known as California Dolphin Swim School) class/program. Where the swimmer is under 18 of age, “Swimmer” herein shall mean Swimmer’s parent or guardian (“Parent/Guardian”), where appropriate.
