 By submitting this agreement, you have authorized the club to bill your bank account for your monthly dues. Your account will be billed on or shortly following the 17th of each month for the amount of $19.95 plus applicable taxes. A $15 plus applicable taxes service charge will be applied for each month dues are returned uncollectible.
