THIS AMENDMENT TO THE AGREEMENT, entered into this day of , 20 , by and between the COUNTY OF SAN MATEO, hereinafter called "County," and Horizon Services, Inc., hereinafter called "Contractor"; W I T N E S S E T H: WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof; WHEREAS, ON August 16, 2006, under Agreement # 74100-07-CO75, the parties entered into a Fee For Service Agreement; and WHEREAS, the parties wish to amend the Agreement to add additional services; and increase the amount of funding the CSAT HIV Door-to-Treatment Grant by $47,880, for a new total obligation of $140,850; and to extend the term to 12/31/2007. NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS: 1.
