The information is vital so that we can find a suitable match for your dog's disposition, age and health. Use a separate Relinquishment Agreement for each dog/puppy. Please give us the dog's veterinary records and collars, tags, bowls, toys, a favorite blanket, and a week's supply of current food when you surrender the dog, to help it acclimate to its new surroundings.
