This residency contract (“Contract”) is entered into this day of between Regions Hospital, a Minnesota nonprofit corporation located at 640 Jackson Street, St. Paul, Minnesota 55101, (“Hospital”), and , residing at (the “Resident”), and sets forth the terms and conditions of Resident’s appointment by the Hospital as a member of Hospital’s staff (“Staff”) assigned to the department of (the “Department”).
