This Local Exchange Telecommunications Services Resale Agreement (“Agreement”), is effective as of the day of 199 (the “Effective Date”), by and between Cincinnati Bell Telephone Company, an Ohio corporation with offices at 201 E. Fourth Street, Cincinnati, Ohio 45201 (“CBT”), and Sun-Tel USA, Inc., a Florida corporation with offices at 10169 Foxcroft Road West, Jacksonville, Florida 32257 (“Sun-Tel”).
