The individual executing this document (the “Participant”) is a participant in the Level 3 2016 Wellness Program (the “Program”). The Program includes, but is not limited to, partial reimbursement for gym/fitness expenses, the opportunity to participate in healthrelated activities and events, and the dissemination of general healthrelated information. The Program is a sponsored by Level 3 and encourages its employee and their spouses/domestic partners to exercise and become healthier. In exchange for participation in and completion of certain Program requirements, employees and their spouses/domestic partners may also qualify for incentives or rewards.
