This Provider Participation Agreement (“Agreement”) is entered into by and between the Colorado Department of Health Care Policy and Financing (“Department”), its Fiscal Agent for the Colorado Medical Assistance Program, and Provider (“Provider”), collectively “the Parties.” This Agreement is entered into in order to define Department expectations of providers who perform services and submit billing, transactions, and/or data to the Colorado Medical Assistance Program. This Agreement is also established to facilitate business transactions by electronically transmitting and receiving data in agreed formats; to ensure the integrity, security, and confidentiality of the aforesaid data; and to permit appropriate disclosure and use of such data as permitted by law.
