Instructions: An original and one copy of this agreement must be completed and signed by the Family Day Care provider1 and returned to the sponsoring organization. The SO must sign both forms, keep one and return one to the provider. This agreement shall be permanent until such time as the provider approval is terminated and/or the provider moves to a new address.
