THIS AGREEMENT is made as of the day of , 202 , by and between (“One Chart Link Partner”) and SANFORD (“Sanford”) for the purpose of granting One Chart Link Partner secure access to select Sanford patient medical records through Sanford’s One Chart Link software (“Service”) as described herein and incorporated into this Agreement.
