Service Level Agreement for On-Net Data Service(s) Addendum QUOTATION NUMBER(S) PAGE OF ACCOUNT NAME (AS LISTED ON COMMERCIAL SERVICE AGREEMENT AND ORDER FORM PAGE 1) ACCOUNT # This addendum sets forth the Service Level Agreement (hereinafter "SLA") applicable to the data services as provided by Midwest Telecom of America, Inc. (hereinafter "MTA''). Except as otherwise set forth herein, capitalized terms shall have the definitions assigned to them in the agreement. MTA shall have sole responsibility for the MTA service and its compliance, or failure to comply, with the SLA.
