AGREEMENT (the “Agreement”) is entered into and effective pursuant to the terms of the Agreement, by and between COMMUNITY CARE OF NORTH CAROLINA, INC., a North Carolina nonprofit corporation, on behalf of itself and its Affiliates (collectively “ CCNC”) and Participants, as defined herein, on behalf of itself and any Participating Entities (collectively, “Participant”) (individually a “Party” or collectively the “Parties”).
