This PARTICIPATING PUMP TEST COMPANY ELIGIBILITY APPLICATION and AGREEMENT (“AGREEMENT”) is made and entered into by ____________________________ (hereinafter referred to as “COMPANY”) to become a Participating Pump Test Company eligible to participate in the Advanced Pumping Efficiency Program (hereinafter referred to as the “PROGRAM”) administered by the California State University, Fresno Foundation (hereinafter referred to as the “FOUNDATION”) under a contract with Pacific Gas & Electric Company (hereinafter referred to as “PG&E”). Once accepted by the FOUNDATION, this AGREEMENT shall bind COMPANY as follows:
