You have chosen to enter into a therapeutic relationship with me. This Client-Therapist Agreement consists of Office Policies; Costs Outside of Customary Services Provided; and a Confidentiality HIPAA Statement. I am providing this information to clarify my policies and procedures for out-patient psychotherapy sessions. Please read this information carefully and sign at the end of this agreement. Should you have any questions, it is important to discuss them with me. These standards and guidelines are designed with the best interests of both client and therapist in mind. In principal, all expectations reflect my highest regard for mutual respect, safety and personal dignity. If no questions are asked by you, it will be assumed that all aspects of this contract are understood and agreed upon.
