My employer has a contract with (CUSTOMER) or one of its divisions, subsidiaries, or affiliates (hereinafter “CUSTOMER”) to provide services and/or furnish personnel to provide contract site services for CUSTOMER. I enter into this agreement in consideration of the compensation paid for services I will perform for CUSTOMER pursuant to my assignment by my employer or as the services may otherwise be provided under contract.
