These General Terms and Conditions along with any Exhibits, Attachments and Service Orders and Addendums together constitute the “Master Service Agreement” (“Agreement”) is made by and between Midcontinent Communications, a South Dakota General Partnership (“Midco”), with a principal place of business at 3901 N Louise Ave Sioux Falls SD 57107, and Enter Customer Name, with a principal place of business at Enter Customer Address on behalf of itself and its affiliates (“Customer”); each a “Party” and, collectively, the “Parties.”
