This Business Associate Agreement (“BAA”) is entered into and effective as of the effective date of the System Access License Agreement by and between Manifest MedEx, a California nonprofit public benefit corporation (“Business Associate”), and the Participant (“Covered Entity”). Both parties hereby agree to this Business Associate Agreement and are referred to in this BAA individually as a “Party” or collectively as the “Parties.”
