FiledJuly 30th, 2013All students attending Mojave Unified School District will be issued textbooks which must be returned at the end of the school year or when a student withdraws. Students will also be permitted to check out library materials if the student’s parent/guardian checks the “Yes” box below. Your signature acknowledges that you, as the student’s parent/guardian, assume full responsibility for any fees/fines due to overdue, lost, damaged, or stolen textbooks and/or library materials incurred by your student. Your student will NOT be allowed to participate in the library program until this form is completed, signed by both the parent/guardian and the student, and returned to the school.
