This Cardholder Agreement (“Agreement”) explains the terms of your Prepaid Card managed by Greenphire, Inc. and issued by Fifth Third Bank, National Association (“Fifth Third Bank” or “Issuer”) under the laws of the United States and the State of Ohio. The Issuer is an FDIC insured member institution. “Card” means the ClinCard Prepaid Visa issued to you by Fifth Third Bank, National Association. You may be issued a physical plastic card (a “Physical Card”) or a digital representation of a card (a “Virtual Card”).
